Sick notes are required for work absences longer than 7 days. At your initial consultation with your doctor, if your absence is likely to be more than 7 days, then the doctor will provide a sick note for you to send to your employer.
Absences of less than 7 days are covered by self-certification forms. Ask your employer for the relevant form.
